                  We are sorry you had a problem, and we’re eager to help you with your warranty claim!

                  

                  Before proceeding, you will need your order number, which you can find in the confirmation email you received from FoamOrder. Please enter only one order number. If you can’t find your order number, please reach out to a Customer Service Specialist to assist, at (415) 503-9314.

                  

                  To start your claim, please:

                  2. Subject: Warranty Claim / Order #
                  3. Name and Phone Number
                  4. Provide photos of depression, following these guidelines:
                  • Remove foam from cover or case
                  • Use a flat surface to place over the depression/indentation
                  • Use a measuring tape showing the depth
